Our End-of-Life Care Process

Our doctor will visit the patient either at home or in hospital to conduct a comprehensive palliative assessment. With access to the patient’s eHealth record, we ensure all care decisions are medically sound and personalized.

We can arrange for the patient’s transfer home via ambulance, accompanied by a doctor and nurse, allowing the patient to begin peaceful care surrounded by family.

Our palliative care medical doctor will conduct routine home visits to manage symptoms with appropriate treatments (e.g., IV fluid or medication, oxygen use , opioid prescription, antibiotics prescription), ensuring comfort throughout the final stages.

Nursing Visits
Registered nurses visit to perform vitals checking, wound care, administer medications as prescribed, and update the care team, ensuring continuous monitoring.

Official Confirmation of Death
When the time comes, there is no need to notify the police. Simply contact our medical team. Our doctor will visit the home to confirm passing and issue the Medical Certificate of the Cause of Death and Cremation Certificate. We work with trusted funeral partners to assist with official document collection.

Our partner funeral home will arrange discreet body transport using a specially modified vehicle (e.g., 7-seater van), ensuring no disruption to neighbors or community.
